Patient Visit Guide

We’re committed to making your visit as smooth, comfortable, and informed as possible. Below you’ll find everything you need to prepare for your appointment.

Your first visit

Your initial appointment will include a consultation to explain your diagnosis and treatment options. In many cases, treatment can begin the same day. If you would like to start treatment immediately following your consultation, please let us know when scheduling.

What to bring with you

  • Referral slip and recent X-rays (if applicable)
  • A list of medications you are currently taking
  • Your dental insurance card and photo ID

Important notes

  • Patients under 18 must be accompanied by a parent or guardian.
  • Please let us know in advance if you have any medical conditions (such as diabetes, high blood pressure, artificial joints, heart conditions, etc.), or if you are taking medications that may affect treatment.
  • If your dentist has recent X-rays, please ask their office to forward them to us. If additional images are needed, we can take them here.

Patient forms

Save time by completing your paperwork before your appointment.

Financial policy

We’re here to help you understand your dental insurance and financial options.

  • Dental insurance is different from medical insurance. Your benefits are based on the agreement your employer negotiated with your carrier.
  • At your visit, we’ll provide an estimate of coverage based on the information available to us.
  • As a courtesy, we file all insurance claims for you. However, the patient remains financially responsible for all treatment fees.

Have questions about costs or financing? Please contact us before your treatment so we can review your options together.

Scheduling policy

If you need to change your appointment, please provide at least 48 hours’ notice so we can accommodate other patients in need of care.

Insurance plans accepted

We work with a variety of dental insurance plans, including Aetna, Cigna, Delta Dental, Guardian, MetLife, United Healthcare, Carrington, and CSDA.

Not sure if you’re covered? Our team will verify your benefits and guide you through your options so you can focus on your care, not the details.

  • During office hours: Call us right away. We’ll prioritize your care and see you as quickly as possible.
